How a referral works
No lock-in contract. We do need a Service Agreement and Schedule of Supports, and we complete that with you at the home visit.

The home visit
We will arrange a time for our Client Care Coordinator to complete a no-cost home visit, get to know you, and understand your goals and support needs.

Together we create your Service Agreement and Schedule of Supports, tailored to your NDIS plan and personal goals.
